Job summary A fantastic opportunity has arisen for an enthusiastic pro-active person to work as a Discharge Planning Assistant within the Complex Discharge Team. We are looking for someone who is able to work on their own initiative as well as part of the great team. support the wards to discharge their patients in a timely manner. As the discharge support assistant the roll is to improve outcomes for patients being discharged once medically fit. Early identification of potential needs and additional support is very important in achieving a short length of stay and reduces risk of potential readmission. The ideal candidate will have a good understanding of the discharge process and be aware of some of the challenges in relation to facilitating good and safe discharge. An ability to demonstrate trust values and behaviours. Have relevant previous hospital experience and a desire to progress in health care. Will have or be willing to work towards completing training that enhances knowledge and skills. Have a flexible approach to meet the needs of the service. Main duties of the job To work with the Complex Discharge Discharge Sisters and Matron to promote a discharge ethos amongst the wider MDT, supporting the base wards to ensure that discharges are focused, timely and safe.
