Job overview
To work within the Mental Health Urgent Care Service’s Home Treatment Team (HTT) to undertake all aspects of clinical duties as a clinical lead within the team. To work with patients with particularly complex needs. The aim of HTT is to provide patient-centred intensive care and treatment to patients and their families at home as an alternative to hospital admission.
Crisis care is a highly specialist intervention and involves supporting our most vulnerable patients and their families in the least restrictive setting possible, by providing a variety of recovery focused psycho-social interventions that enable the person to regain or maintain independence.
Main duties of the job
1. Ensure that the care delivered is excellent and in keeping with the most up to date guidelines and best practice.
2. Work with carers/family members as necessary to provide a thorough bio psychosocial assessment of their needs.
3. Take an advisory and supervisory role in partnership with the clinical team in managing complex cases.
4. Lead and present complex, sensitive, and challenging clinical and professional communication processes.
5. Safeguard the health and wellbeing of children and vulnerable adults by adhering to Trust safeguarding policies.
6. Utilise problem solving and decision-making skills to support safe discharge planning.
7. Regularly deal with difficult and distressed patients and carers, conveying complex information.
8. Provide a modelling role to all staff; ensuring standards of care are met.

Person specification
Qualifications
* Registered Mental Health Nurse.
* Demonstrable experience and clinical expertise in managing individuals who are acutely unwell.
* Clinical supervisory experience and willingness to provide clinical supervision.
* Demonstrable evidence of leading on development of care packages to meet holistic needs.
* Experience of working across organisational boundaries and developing multi-professional partnerships.
* Evidence of advanced practice clinical assessment and treatment skills.
* Experience and ability to engage service users and carers in risk management and care planning.
* Relevant experience within Crisis Home Treatment Service.
* Delivering crisis skills/treatment interventions to individuals who would otherwise need hospital admission.
* Experience of working as a clinical lead within an adult setting.
* Experience of leading reflective learning sessions with staff team.
