We are seeking to appoint a Band 6/7 (depending on experience) Diabetes Specialist Nurse on a permanent part-time (22.5 hour) basis. This role will be based at our Broomfield Hospital site.
You will work as part of a specialist MDT team to provide a specialised diabetes service for hospital in-patients and outpatients, supporting a caseload of patients utilising technologies such as CGM, CSII, and Hybrid closed loop systems.
You will provide clinical advice and support through education, training, and health promotion for patients and their families/carers as appropriate, and act as a resource for other HCPs such as practice nurses, doctors, the ICT, and ward clinical staff in relation to the development of clinical management plans for patients.
Main duties of the job
1. To educate and empower patients with diabetes to self-manage, working in collaboration with the other DSNs, consultants, and PAMS.
2. Assess, plan, and implement nursing care for a caseload of patients with diabetes, working in collaboration with consultant colleagues.
3. Provide a high level of communication by liaising with all appropriate members of the multi-disciplinary team.
4. Work within the remit of non-medical prescribing.
5. Obtain appropriate consent for all procedures.
6. Challenge inappropriate practices that impact on the patient/others/Trust.
7. Maintain an environment that actively and consistently recognises the patient's rights, including the right to confidentiality, in line with the Data Protection Act and GDPR.
8. Commit to sharing good practice across the Trust.
9. Communicate sensitive information to patients and relatives as appropriate.
10. Able to use and teach others how to use a wide range of specialist equipment such as continuous blood glucose monitoring equipment.
