This is an exciting opportunity for an experienced innovative and enthusiastic nurse to join our Colorectal Cancer Specialist Nursing Team.
The post holder will work closely with the surgical and oncology team and the wider Multi-disciplinary Team (MDT) and be senior member of the Colorectal Cancer Specialist Nurse Team.
The post holder will play a pivotal role in supporting colorectal cancer patients along clinical pathways to continually improve the patient experience. This nurse should be motivated, dynamic and possess excellent interpersonal, communication and organisational skills and be committed to working as part of the Colorectal Team at ELHT.
Main duties of the job
A Clinical Nurse Specialist in colorectal cancer provides expert advice, information and support for patients and has the knowledge and skills necessary, to develop and maintain, effective therapeutic relationships, over a period of time. We require someone who can work both autonomously and as part of a team, and demonstrate leadership, innovation and be self-motivated.

Working as a member of the Colorectal Team the post holder will provide specialist nursing care to patients with a diagnosis of colorectal cancer. They will be responsible for the co-ordination of the patients’ treatment pathway, acting as a link between primary, secondary and tertiary care. They will act as a named key contact for patients, relatives and carers on behalf of the colorectal cancer MDT and provide support for patients and carers from the time of diagnosis onwards
Detailed job description and main responsibilities
The post holder will be expected to Undertake Holistic Needs Assessment and implementation of subsequent plans of care agreed with the Patient and if relevant their family/carers.
Be responsible for the safe and effective care of colorectal cancer patients by effective assessment of complex needs which may require specialist intervention.
Act as patient advocate to ensure the delivery of care is focused on the colorectal cancer patient and family
Support relevant clinical and non-clinical staff across hospital wards and departments and community care settings with regard to colorectal cancer issues and agendas
Proactively support the wider MDT with planning, implementing and evaluating care
Support in the tracking of patients to ensure timely access to agreed treatment plans ensuring organisational compliance and achievements of cancer performance targets
Support in the developments and delivery of nurse led risk stratified follow up and patient Supported Self-Management
Work closely and collaboratively with HPB team to provide ongoing cover for both services.

Qualifications
Essential criteria
* Registered General Nurse
* Has a degree in health related subject or equivalent or equivalent experience
* Post-registration qualification in colorectal nursing/oncology or willing to undertake
