JOB POSITION Job Title: Practice Nurse Department: Nursing ORGANISATION STRUCTURE Responsible to:Nurse Manager JOB PURPOSE To provide nurse-led care of patients: maintaining current health and providing intervention in acute or chronic conditions. To demonstrate clinical expertise, working with all members of the Primary Health Care Team and other Healthcare Professionals where appropriate. To be committed to self-development and professional growth through participation in continuing education. MAIN RESPONSIBILITIES AND TASKS 1. Assess and identify the nursing care required by the patients. 2. Provide the relevant nursing care, to plan that care and set achievable and agreed goal with the patients. 3. To educate the patients and provide relevant literature and support. 4. To be responsible for maintaining and updating professional knowledge, education and professional registration 5. To work to agreed guidelines and protocols and participate in their development. 6. To keep up to date computer or hand held records of patient care. 7. Maintaining a safe environment for patients and colleagues. 8. To provide supervision and education for junior staff members. To Provide: 1. Health promotion 2. New patients checks 3. Childhood immunisations and vaccinations 4. Smoking cessation advice and support 5. Other vaccinations are required by patients and national campaigns 6. Well person checks 7. Assistance to GPs as required 8. Nursing care and advice for minor injuries and ailments To provide specialist-nursing care in the following areas as appropriate: 1. Cervical Cytology 2. Warfarin Initiation/INR Clinic 3. Chronic Disease Management 4. In House training to Junior nurses/HCA 5. Travel advice and vaccination 6. Contraception and sexual health 7. Spirometry General treatment room duties will include: 1. Checking treatment room drug expiry dates 2. Ensuring that the treatment room equipment is maintained and up to date and stocked up 3. Assisting with minor surgery, iud, implant insertion as required 4. Display of health promotion materials in conjunction with nursing team General Nursing care will include: 1. Wound care 2. Ulcer care 3. Removal of sutures 4. ECG 5. Use of emergency resus equipments. 6. Zoladex injections 7. Minor eye problems 8. Dietary advice and education 9. Immunisation and vaccination 10. B12 injections 11. Hormone injections 12. Swabs 13. Telephone consultations 14. Minor ailments e.g infected wounds etc 15. Urine testing. Prescribing (where appropriate) If you are a Nurse Independent Prescriber (NIP) then the following applies: You will have valid registration with the appropriate professional body You will prescribing with the scope of training and knowledge You will liaising with mentor/GP colleague where support is required Administration 1. Record keeping 2. Communicating effectively with other elements of the PHCT and outside agencies where appropriate 3. Attending meetings 4. Attending relevant educational courses and maintaining education 5. Audit 6. Responsibility for clinical areas of QOF 7. Any other appropriate and relevant tasks as requested by the senior nurse. The above tasks are not exhaustive and the practice may require additional reasonable tasks to carried out from time to time.
