Job summary The focus of the Care Coordinator role is to create opportunities to improve the patient experience and ensure patients are directed to the best source of help for their needs and circumstances in a timely manner. This post requires communication with multiple agencies outside of Seaford Medical Practice and also multi-disciplinary working within the practice. Also within the scope of this role is a contribution towards the quality of medical records. Together, all aspects of this role require a sound understanding of medical terminology and of the patient experience. About us Seaford Medical Practice has a clinical team consisting of 6 GP Partners, 5 salaried GPs, 3 Sisters, 5 Practice Nurses, 6 HCAs, 1 Phlebotomist, 2 Paramedics, 3 ACP's, Clinical Pharmacist, First Contact Practitioner Team. The patient list is currently 18k patients. Person Specification Qualifications Essential Qualification and Experience: Previous experience in healthcare or care coordination, ideally within a GP/Clinical setting.
