Job Overview

An outstanding opportunity has emerged within our award-winning Integrated Community Cluster Network, and we're searching for a dedicated professional to join our team. In this role, you'll be at the forefront of community care, providing essential support for individuals with progressive neurological conditions and offering universal palliative care.
Main Duties
1. To act as a specialist independent practitioner in the systematic autonomous provision of Speech and Language Therapy services for adults with speech, language, communication and swallowing difficulties in a multi-disciplinary team.
2. To be responsible for the speech and language assessments, differential diagnosis, treatment and discharge of individuals within these well-resourced networks.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Qualifications/Experience
Essential criteria:
* Relevant SALT qualification
* Registered SALT HCPC
* Appropriate SALT clinical skills
* Experience of working with other professionals
* Evidence of working in a person-centred and outcome-focused way
* Experience of case load management and post-qualifying experience
* Good verbal and written skills
Desirable criteria:
* Experience of working in the community
* Experience of mentoring others
