We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Full Sutton, a category A men's Prison located in Full Sutton.
This site is located in a rural location with their own parking on site, an on-site gym, caf, team building days and well-being events annually, regular 1-1's and good scope to develop into a Senior Practitioner.
Hours
hours a week - Day shifts Monday - Sunday (some night cover required). On a rolling rota.
Salary
This position is full time, youll receive an annual salary up to £40,211per annum FTE - depending on experience.

Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
1. Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
2. Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
3. Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
4. Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
5. Deliver non-judgemental care that makes a real difference
6. Inspire excellence by forging strong links with our partners and stakeholders
7. Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
8. Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:
16. Registered nurse Adult
17. Current professional qualification (NMC)
18. Evidence of Continued Professional Development
19. Understanding of the complexities of multi-disciplinary and multi-agency working
20. A non-judgmental and compassionate approach
21. Excellent communication skills
