Who You Are You are a qualified and experienced social worker with a minimum of two years post-qualification experience in statutory Adult’s Social Care services. Holding a degree in Social Work or equivalent, and registered with the Health and Care Professions Council (HCPC), you demonstrate a deep understanding of the NHS and Community Care Act, Mental Health Act, and other legislative frameworks, statutory guidance, and processes for vulnerable adults. Your expertise extends to needs-led assessments, care planning, and direct work with individuals and families. Known for your excellent organizational skills, you successfully manage a fluctuating caseload, delivering person-centered, outcome-focused reports and plans. You're recognized by peers for your critical knowledge of social work intervention theories and models and are adept at teaching and assessing social work students. Strong in communication and personable, you ensure collaborative outcomes, applying reflective and analytical skills to your casework and professional development. What the job involves Your role involves actively contributing to the delivery of an effective Vulnerable People Service, managing a complex and varied caseload, conducting thorough assessments, and crafting support plans tailored to individual needs. You'll engage with legislative frameworks, preparing statements and representing cases in court as necessary. Your responsibilities include identifying risk and need levels, conducting safeguarding investigations, and working collaboratively with multiple agencies to negotiate and coordinate personalized care packages. You'll engage in direct work with adults and families, chair multi-disciplinary forums, and support the improvement of both practice and service delivery. Managing data entry and ensuring confidentiality under the Data Protection Act are essential, complemented by a commitment to health, safety, and equality policies. As a specialist, you'll oversee social work student placements, participate in professional supervision, and assist in service development while operating a hybrid working model combining office attendance and remote work.
