An exciting opportunity has arisen within the Cystic Fibrosis Adult Service at Liverpool Heart and Chest Hospital. We are looking for a motivated, enthusiastic and experienced nurse to help deliver outstanding care to people with Cystic Fibrosis. You will be an integral part of a large MDT team providing in patient, out patient and community care.
Main duties of the job
The post holder will be required to:
1. Be an integral part of the multi-disciplinary team providing professional and clinical support for the care of people with cystic fibrosis.
2. Assist in providing specialist advice, care, support and education for families and carers of people with a diagnosis of Cystic Fibrosis and associated co-morbidities such as CF diabetes.
3. Participate in the enhancement of the Cystic Fibrosis service that will cover the whole patient journey.
Job responsibilities
CLINICAL
1. Be accountable for the direct delivery of care to a caseload of patients assisting in carrying out comprehensive assessments in order to plan, implement and evaluate care delivery.
2. Demonstrate clinical knowledge of skills in respiratory medicine in relation to the assessment, co-ordination and facilitation of admissions to hospital, instigation of home IVs and management of in and outpatient care.
3. Comprehensive assessment, planning, implementation and evaluation of care within a variety of settings.
4. Awareness of the social and psychological impact of the disease and involvement in assessment of these aspects of care.
5. Awareness and implementation of safeguarding measures.
6. Assistance with co-ordination of care for people with CF Diabetes.
7. Supporting the transition of new patients from paediatric services over to adult care.
8. Assistance with co-ordination of care for patients referred and accepted for transplantation.
9. Co-ordination of care and support for people with CF towards their end of life.
10. Demonstrate an awareness of research and evidence-based practice to improve patient centred care.
MANAGERIAL/LEADERSHIP
1. Demonstrate professional leadership and assist in the writing of both clinical and developmental policies, protocols and procedures for this service.
2. Liaise with other nursing staff to promote teamwork and ensure equality of service provision across the Trust for people with Cystic Fibrosis.
3. Evaluate service provision by participating in audit and implementing action plans based on the results.
4. Maintain safe standards of practice at all times and report any concerns to a senior manager/team member.
5. Keep and maintain accurate records and ensure that confidentiality of information is always assured.
ORGANISATIONAL / EDUCATION
1. Be responsible for identifying own training needs and maintaining own professional development.
2. Use specialist knowledge to assist and support with teaching relating to Cystic Fibrosis, to staff and students.
3. Support students and act as a mentor when required.
4. Continually update nursing practice in accordance with current research and evidence.

Experience
1. Extensive experience working in Cystic Fibrosis Care
2. Understanding of multi-organ involvement of CF implications for clinical care
3. Willingness to undertake further training in Cystic Fibrosis care.

2. Evidence of understanding and application of clinical audit and research
Skills
1. Excellent interpersonal and communication skills
2. Ability to work as part of a team and across the Trust.
