Job summary Advanced Mental Health Pharmacist: Winchester Rural North & East, Winchester Rural South and A31 Are you an experienced pharmacist who is passionate about mental health and looking for a new challenge? The Southern Health Medicines Management Team has secured funding as part of the Community Mental Health No Wrong Door Transformation Programmed to recruit full time mental health pharmacists to work across Mid and North Hampshire linking in with defined PCNs. These posts will establish a new clinical pharmacy service working across the primary/secondary care interface supporting designated Primary Care Networks (PCNs). Successful applicants will integrate within clinical teams as part of the multi-disciplinary approach delivering medicines management services for patients and improving processes to reduce risks with medicines especially between different care settings. We are looking for experienced, compassionate, confident, enthusiastic and motivated pharmacists who are passionate about improving patient care. You will join our friendly, forward-thinking, progressive and expanding Medicines Management Team that provides support to mental health and community services across Hampshire. Main duties of the job This role will support the Community Mental Health Transformation project by providing specialist mental health pharmaceutical support to patients of the Winchester Rural South, Winchester Rural North and East and A31 Primary Care Networks, working within primary care, professionally supported by Southern Health.
