As an Advanced Nurse Practitioner, you will need to manage your own patients in a primary care practice setting. Telephone triage may also be required. You will be required to fully document all aspects of patient care using the EMIS Clinical System and agreed templates and protocols, working in accordance with agreed administrative systems. You will need to send and receive written information on behalf of the practice relating to the physical and social welfare of the patients.
Work closely with the manager and other members of staff in setting up and improving practice systems for monitoring and measuring performance against clinical governance and quality indicator targets, including those relating to the quality and outcomes framework and enhanced services. In summary, the successful candidate will:
1. Have experience in General Practice
2. Have active registration with NMC
3. Keep your registration and training up to date and source relevant training
4. Be educated to master's level in advanced nursing
5. Be an independent prescriber
6. Meet NMC revalidation requirements
7. Demonstrate autonomous evidence
8. Follow agreed policies and procedures
9. Act upon alerts (e.g., QOF) during routine visits
10. Provide all ANP services required
11. Provide appropriate healthcare for patients presenting with health problems
12. Provide preventative healthcare and advice
13. Provide medical support for patients referred by colleagues
14. Adhere to the practice prescribing formulary, local and national guidelines
15. Record clear IT-based consultation notes to agreed standards
16. Collect data for audit purposes and participate in clinical audit
17. Attend and participate in appropriate meetings as deemed necessary by the PM, e.g., MDT and clinical meetings
18. Carry out any other duty deemed necessary for the safe and smooth running of the practice
