Bromley Healthcare are currently recruiting for Bank Band 5 Community teams consist of District Nurses, Community Nurses, Nurse Associates, Apprentice Nurses, Healthcare Assistants and Phlebotomy. They are a busy & dynamic team who have patient care at the heart of all they work is crucial to the community as we aim to prevent unnecessary hospital admission for our patients.

Main duties of the job
The Community Nurse role is a highly skilled role which involves skills such as Picc lines/IV's as these are skills that are very much key to being an excellent Community Nurse.
As a Community Nurse you will deal with a variety of patients in their own homes, the role will involve but is not limited to:
1. Responsibility for contributing to the holistic assessment, planning, delivery and evaluation of patient care.
2. Implementing, monitoring and maintaining our high standards of care.
3. Re-assessing and re-evaluating programmes of care when needed.
4. Supporting our in-house mentorship training scheme with placement students and staff.
5. Being an active member of the team sharing best practice whenever possible.
This is a challenging but ultimately rewarding position where you will be supported both within your teams and by regular reflection & professional training.

Please note that all Community Nursing roles are designated essential car users therefore applicants should have a current driving licence and access to a vehicle to be considered for this position.

Job responsibilities
To contribute to the clinical delivery of the service. This will include completing holistic patient assessments, designing and regularly reviewing care plans, delivering effective clinical care.
To assist with the daily coordination of the team to include daily handover, allocation of work and managing referrals and telephone develop good working relationships with Allied Health Professionals, specialist and other clinical services in Bromley Healthcare and contribute to the development of policies and procedure that cover related services.
To proactively monitor and manage a daily patient caseload making autonomous decisions about patient care and seeking support from colleagues as appropriate.

7. Current NMC Registration (or awaiting confirmation of qualification and registration at time of application)
8. Preparation for mentorship/ENB 998/ equivalent or willing to undertake.

11. Evidence of provision of high standards of clinical care.
12. Valid UK Driving Licence and the use of a vehicle for work purposes and the ability to competently use a 'satnav'.
