We have an exciting opportunity for an independent, autonomous Prescribing Pharmacist to support our Urgent Care service within the out of hours (OOH) period on weekday evenings and weekends. You will work as part of Medvivo’s multidisciplinary team at our Chippenham head office to deliver safe and timely care to those accessing our Integrated Urgent Care Service.
About the role:
Our Prescribing Pharmacists work as part of our multidisciplinary team from our call centre in Chippenham to deliver telephone triage of patients for our GP Out of Hours service. You’ll manage calls referred to us from the 111 service relating to medication queries and repeat prescriptions. There is always the support of one of our Advanced Clinical Practitioners or GPs during your shift, either working alongside you or remotely.
You will assist in the development of an integrated approach to the delivery of patient care across the ‘whole system’ of health and social care, working in partnership with all relevant stakeholders.
We provide a bespoke induction and ongoing continuing professional development. This is a role which we see evolving as the service grows and we would look to support our team of Prescribing Pharmacists to build their telephone assessment decision making skills to enable them to deal with more complex 111 cases. Clinical outcomes may include closing the case with appropriate worsening advice, a short term course of medication or referring for a face to face assessment at a base or home visit.
Work pattern:
We are seeking to recruit an autonomous Prescribing Pharmacist to work a variety of shifts based at our head office in Chippenham. Our shift options span across weekday evenings Monday-Friday (18:30-23:00) and also on weekends (08:00-13:00, 13:00-18:00, 14:00-19:00 and 18:00-23:00).
Requirements:
* Current GPhC registration.
* MSc in Advanced Practice or evidence of accumulation of relevant experience knowledge.
* Non medical prescribing qualification.
* Ability to demonstrate a wide knowledge base and competency in clinical practice.
* Understanding of the emergency care pathways.
* Knowledge of primary health care issues affecting a diverse local population.
