A new and exciting key post has arisen for an enthusiastic registered nurse to join the All Age Continuing Care team in Cheshire.
We are part of the Nursing and Care Directorate of Cheshire and Merseyside ICB, and you will play a key role in helping our award-winning service, deliver key aspects of The National Framework for Continuing Healthcare. Cheshire and Merseyside ICB are committed to providing patients with the best level of care and there are exciting times ahead for All Age Continuing Care teams across the region.
The successful candidate will work across Cheshire and be flexible to meet the needs of the service. We operate a hybrid working model (blend of home and office working), however there may be occasions where travel outside of Cheshire may be required.

Main duties of the job
The post holder is responsible for the clinical decision making and the process and oversight of all of the screening and triaging for referrals that come into the Patient Access Team (Single Point of Access). They will need to use specialist knowledge of National Continuing Healthcare and Funded Nursing Care Framework and Mental Health Act to ensure referrals flow into appropriate assessment pathways. The post holder will make quality assurance decisions in line with operating procedures. They will be responsible for ensuring prompt and accurate assessment and approval of commissioned packages where appropriate.
The post holder is responsible for overseeing the management of all complaint's pathways for the service. Will work on Appeals and Previously Unassessed Period of Care.
The post holder will lead the resolution of Responsible Commissioner disputes with out of area organisations.
The post holder will lead a team delivering Continuing Healthcare ensuring that staff adhere at all times to the NHS Continuing Healthcare National Framework and work within established policies, procedures, protocols and systems, ensuring quality standards and performance.
They will be responsible for effective communication with a range of individuals and will need to use tact, diplomacy and conflict management skills to inform, advise and sign post.

Job responsibilities
1. To use expert knowledge of National Continuing Healthcare and Funded Nursing Care Framework and Mental Health Act (117 aftercare) to ensure appropriate referrals to the Service;
2. To provide patient focussed service by ensuring prompt and accurate and effective decisions within the single point of access of NHS Continuing Healthcare Services;
3. To use Who pays Determining Responsible Commissioner guidance to determine appropriate patient referrals;
4. Responsible for the verification of Continuing Healthcare decisions on behalf of the ICB, in line with operational procedure and organisational policy;
5. To ensure the single point of access function is delivered in line with national guidance and best practice;
6. Overseeing the management resolution of complaints, appeals and disputes, ensuring operational staff complete to a high standard;
7. Lead the resolution of Responsible Commissioner disputes with out of area ICBss or other organisations; liaising with legal and Senior ICB leadership when appropriate.
8. Lead and manage staff as requested ensuring a high quality service is delivered, supporting staff development and continuous improvement in line with HR policies;
9. Contributing to performance improvement, taking a lead for identified areas where agreed, and overall day to day management of performance in their area;
10. To be aware of and implement functions and responsibilities around the Mental Health Act and 117 aftercare;
11. Support the analysing, interpreting and communicating complex national directives into processes, policies and presentations for application;
12. Actively supporting the team in working towards goals and objectives and investigates the causes of any variance from target/plan and proactively contributes to the implementation of solutions;
13. Communicating information, risks, issues and dependencies, including briefings and reports;
14. Maintaining up-to-date knowledge of legislation, policies and guidance in line with appropriate healthcare sector and ensuring this is disseminated appropriately and in a timely manner to the clinical team;
15. Accurately input information onto departmental databases and tools used in Continuing Healthcare adhering to NMC standards for record keeping;
16. To work autonomously, being accountable for own actions and decisions without direct supervision or support;
17. To prevent care package crisis by supporting in a timely and appropriate manner, through good decision-making and sign posting to other services where appropriate;
18. To work collaboratively with the colleagues and multi-disciplinary professionals within the Service and other agencies to support complex decision-making for referrals.
19. Attend and participate in agreed meetings, training, development and supervision sessions as an individual and as part of the team;

23. To ensure any staff that are line managed by the post holder receive appropriate supervision;
24. Ensure own and any direct reports compliance with all statutory and mandatory training;

27. Educated degree level or equivalent level of experience of working in specialist area.
28. Current NMC Registration.
