Job Description
We are seeking a permanent Social Care Practitioner within the Short Term Assessment and Reablement team in South Derbyshire.
Applicants should have experience working in an Adult Social Work setting and demonstrate a person-centred, strengths-based approach to help individuals live as independently as possible within their own homes and communities. Experience collaborating with statutory and voluntary organizations is essential to support clients in returning home or remaining at home after periods of ill health or changing needs.
The successful candidate will need knowledge of relevant legislation, including the Care Act 2014 and the Mental Capacity Act 2005, as they will assess care and support needs to promote independence.

The role may include unsocial hours, including weekends and Bank Holidays. Payment details will be provided in the contract.
Weekend hours are paid at standard rate unless they fall between 7pm and 7am, when enhanced rates apply. Bank Holiday work also attracts enhanced pay.
