What the Job Involves The initial four weeks involve a comprehensive induction program from Monday to Friday. Post-induction, the role requires office presence twice a week. Your main responsibilities include conducting assessments, developing support plans, identifying necessary resources, and performing regular reviews to foster the independence of adult social care service users. You will also complete assessment documents, manage records, prepare reports and correspondence, and determine when referrals to specialists are necessary. Additionally, you will coordinate and review ongoing support packages and act as a care coordinator for service users when needed.
