Full job description
Purpose: Responsible for the delivery of care and for managing service provision in accordance with national and local priorities, organisation policies, procedures and guidelines and in accordance with the Nursing and Midwifery Council.
Principal duties include:
1. Participate in assessment and care planning of patient care in conjunction with the District Nurse.
2. Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan; escalate any deviation from the care plan or change in condition to the District Nurse / GP accordingly.
3. Communicate effectively with patients, carers, and all members of the multidisciplinary team.
4. Prioritise daily work in line with clinical / patient need.
5. Maintain accurate and legible patient records in line with professional and organisational requirements, including contemporary paper records and electronic patient record systems.
6. Implement and work to ensure evidence-based care / best practice.
7. Actively participate and contribute to team meetings and daily handover.
8. Initiate health promotion activities with individuals, patients, and carers.
9. Take responsibility for a defined caseload in the absence of the District Nurse, under the direction of the Practice Teacher / Team Leader.
10. Be actively involved in the development and implementation of community nursing service developments and initiatives.
11. Participate in clinical audit and the implementation of changes to ways of working / clinical practice as a result of findings, recommendations, and action plans.
12. Participate in risk assessment and promote the safety and well-being of patients, staff, and any other persons at all times, assisting in ensuring a safe work environment.
13. Ensure that all complaints, untoward incidents, accidents, and areas of clinical risk are identified and reported to the District Nurse in accordance with the organisation policies.
14. Keep up to date with professional requirements, healthcare developments, clinical evidence, National Service Frameworks for practice, and National and Local policy and guidelines.
15. Be responsible for completing all own mandatory training within the required timeframes.
16. Access and participate in regular clinical supervision and peer review.
17. Participate in own annual appraisal and undertake own continuing professional development in order to maintain and develop knowledge and skills.
18. Access regular 1-1 meetings with the District Nurse.
19. Undertake mentorship training and facilitate the teaching and learning experiences for all students and staff.
20. Be accountable for own professional actions in line with the NMC code and organisational policy and guidelines.
21. Monitor and order stock levels and equipment in line with budget and lean principles.
22. Be responsible for the security, care, and maintenance of equipment ensuring standards of infection control and safety are maintained.
