Job summary The role will work within an integrated care system to organise and manage up to 3 Neighbourhoods in a defined locality in Hillingdon (North, Central or South). Using a change management approach and methodology to identify, plan and deliver transformation initiatives to support and enhance Neighbourhood working. Main duties of the job Proactively engage staff within the Neighbourhood teamsand wider stakeholdersto enable delivery of initiatives Committed to working and engaging constructively with internal and external stakeholders on a range of business sensitive issues Foster and maintain strong links with all services across the PCN and neighbouring networks.
