We are seeking to appoint a GPs with an interest in Frailty to support the delivery of the various developing Frailty Services, including but not limited to,
* You will undertake a variety of duties, offering support and advice to System providers regarding vulnerable patients in Community as part of an MDT, undertake best interest discussions with patients and family members and help co-ordinate a response to support people in need to stay home whenever possible.
* More rarely you may be needed to visit a complex frail patient at home or undertake telephone consultations.
* Assessing the health care needs of complex patients with undifferentiated and undiagnosed problems, highlighting which services would support them best to stay in their home setting and independent.
* Cover the clinical management of patients as appropriate including initiating investigations, reviewing results, making referrals to other providers/services as appropriate.
* Compile and issue computer-generated acute and repeat prescriptions.
* Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).
* Work with and support other members of the MDT, including but not limited to: social workers, therapists, social prescribers, voluntary support workers, frailty nurses, dieticians, UCR paramedics and other Frailty GPs.
* Participate in frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.
You will need to hold the relevant professional registration (GMC) and have experience of Triage / Telephone Assessment. Experience of working in Geriatric Medicine is desirable. Any successful candidates would join our growing team of GPwERs in Frailty and be supported to obtain their Diploma in Geriatric medicine as well as join our regular education sessions.

This is predominantly a face-to-face role based at Acomb Garth Community Centre between the hours of 8.30am and 5pm but if doing any evening shifts between 5 and 8pm then this can be done remotely from home (evenings paid at enhanced rates).
