A fantastic, new opportunity has arisen to become a Homeless Recovery Co-ordinator, supporting rough sleeping individuals throughout their treatment journey and successfully supporting their move on into suitable housing arrangements. We are hoping this role will be for you.
You will be the key liaison between CGL services, local hostels and shelters, the local council and partners and will actively engage individuals with the services they need to support improved health, wellbeing, social inclusion and appropriate accommodation.

Full Time Hours: 37.5 per week
Full Time Salary Range: £27,449 - £31,529 dependent on experience, pro rata for part time hours
Allowance: Outer London Weighting of £2,163.27
Contract Type: Fixed Term contract for 12 months
Responsibilities
About the role:
* Establish effective communication and referral pathways for those individuals who are homeless and in contact with other health and care agencies.
* Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
* Coordinate the transfer of any accommodated out of area individual.
* Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting rough sleepers.
* Provide advocacy for access to partnership services.
* Work with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination.
About you:
* Prior knowledge and experience of working with people who are homeless, sleeping rough or sofa surfing.
* Be an excellent communicator, both verbal and written with good IT skills.
* Proactive and able to work on own initiative with excellent time management and prioritising skills.
* Have a good working knowledge of mental health interventions, services, and good practice.
* Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.
* Ability to manage change successfully in a way that prioritises the needs of service users.
* Understand the importance of information governance processes and commit to follow and apply all necessary safeguards.
* Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others.
