For internal candidates the role is suitable for a secondment opportunity or for external candidates a temporary contract. The role involves working with the front door colleagues in A&E to assist with admission avoidance and discharge planning. About The Role About the role As a Social Work Assistant, you will play a crucial role in supporting our social work team in delivering high-quality services to our clients. Your key responsibilities will include: Assisting social workers in case management and client assessments. Conducting home visits and providing support to clients in their homes. Coordinating and facilitating access to community resources and services. Maintaining accurate and up-to-date client records and documentation. Participating in team meetings and contributing to case discussions. Providing emotional support and guidance to clients and their families. Mobile: A ‘mobile’ role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. You’ll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary. About You To be successful in this role, you should have the following experience, knowledge, and skills: Previous experience in a social work or related field is preferred. Excellent communication and interpersonal skills. Ability to work effectively as part of a team and independently. Strong organisational and time management skills.
