We are hoping to recruit an enthusiastic & highly motivated Advanced Nurse Practitioner to join our friendly and growing practice team. We are a 4 site practice providing high quality care to 17,000 patients in the Longton & Meir areas of Stoke On Trent. You will be an autonomous practitioner working independently and in conjunction with other health care professionals. You will assess, diagnose and treat patients with a range of minor illness and injuries, acute and chronic medical conditions and initiate direct referrals as appropriate.
Main duties of the job
The post holder will be competent in general practice using their expert clinical knowledge and skills. They will have the authority to make autonomous decisions in the assessment, diagnosis and treatment of patients.
The post holder must be able to independently prescribe appropriate medication, evaluate or refer to other specialists if necessary and ensure that patients fully understand any procedure to be carried out on them, and that the appropriate consent is obtained and documented in the Emis Web clinical system.

Job responsibilities
The core responsibilities of the Advanced Nurse Practitioner are to:
* Work autonomously and be accountable for his/ her professional actions.
* Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.
* Assess patients with a range of acute, non-acute, and chronic medical conditions. To take history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.
* Where appropriate make referrals to other healthcare professionals in primary and secondary care.
* Arrange hospital admission for appropriate cases.
* Undertake telephone triage work and provide telephone advice calls as appropriate.
* Prescribe medications as appropriate.
* Maintain accurate record keeping in Emis Web.
* Undertake regular medication reviews and medicines management.
* Request and interpret tests appropriate to patient needs.
* Participate in clinical audit.
* Participate & contribute to clinical team meetings.
Person Specification
Qualifications
* Registered Nurse with Nursing and Midwifery Council.
* Masters degree required for qualification post December 2020.
* (See RCN Credentialing for Advanced Level of Nursing Practice).
* Post graduate diploma or degree for Advanced Practice Qualification up to December 2020.
* Qualified Independent Nurse Prescriber on the NMC register.
* Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
* Meets the standards for registered ANP working at advanced level.
* Qualified triage nurse.
Experience
* Experience of working in Primary Care.
* Development of areas of clinical expertise.
* Nurse-led management of minor illnesses and minor ailments.
* Management of patients with long term conditions.
* Nurse-led triage.
* Developing protocols and clinical guidelines.
* Advanced clinical practice skills.
* Minimum 2 years recent experience in a primary care setting.
