Job summary
The Southend East PCN is looking for a part-time pharmacist to expand our pharmacy team. Our team already consists of a lead, 5 pharmacists and 3 technicians. This role would be mainly focused around achieving the MOLES (Medicines Optimisation Local Enhanced Service) targets for the PCN, both clinical work and clinical audit work.

Main duties of the job
Management of medicines after discharge from hospital
Risk stratification (Identify patients at high of harm from medicines)
Unplanned hospital admissions (Identify patients at risk of unplanned hospital admissions from medicines)
Repeat prescribing (Manage repeat prescribing reauthorisation process by reviewing patient requests)
Patient facing medicines support
Care home medication reviews
Long term condition clinics
Service development (Develop and manage new services built around new medicines or NICE guidance)
About us
Southend East PCN is located at Norton Place in Shoebury, along Ness Road. We have our own PCN building which is extremely active and is in use by our 5 member practices in the local area. We cover approx 37,000 patients and offer a range of appointments from our wide range of staff, fully utilising the ARRS budget. Our team consists of pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists and assistant practitioners. We have 2 clinical directors who are partners in two local surgeries, and an operations manager to provide support wherever needed. We have a strong digital presence and have employed a Digital Transformational Lead, to help our PCN network with other businesses and mature over time.

Clinical Responsibilities
Highlight any gaps in knowledge/training needs to the Clinical Nurse Manager to allow them the opportunity to offer support in order to fulfil the duties of this post.
Remain up-to-date with clinical guidelines to ensure that practice remains evidence based.
Ensure clinical practice is safe and effective and remains within the boundaries of Competence.
Work in line with organisational policies and procedural guidelines.
Attend and participate in practice meetings as required
This role will evolve to meet the changing needs of the population and the Practice Pharmacist should be prepared to undertake further training as required in order to fulfil the duties of this post
Clinical governance
Keep up to date with local and national guidelines and the ICB formulary with medications
Ensure accurate and legible notes of all consultations and treatments are recorded promptly in the patients notes on SystmOne.
Ensure the safety of patients, relatives and colleagues and to report all incidents and near misses in line with the risk management policy.
Remain up-to-date with all mandatory training
Maintain up-to-date knowledge of safeguarding adult and children guidance to ensure that those at risk of abuse/suffering abuse are identified and the necessary process to protect them is followed
Meet responsibilities as an employee as set out in the organisations corporate andclinical policies.
Maintain confidentiality at all times.

Qualifications
Essential
1. Mandatory registration with General Pharmaceutical Council
Desirable
2. Membership of the Royal Pharmaceutical Society
3. Masters degree in pharmacy (MPharm)
4. Clinical diploma
5. Clinical assessment skills

6. In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
