We are looking for an enthusiastic Registered Nurse Associate (RNA) to join our innovative Virtual Ward Team as a Discharge Clinician. Main duties of the job
This role is an opportunity for a Registered Nurse Associate with NMC registration and an appropriate Foundation Degree.
1. Assess and prepare patients for discharge from the acute hospital onto the Virtual Ward (VW), ensuring they are ready for a safe transition to community care at the place they call home.
2. Collaborate with multidisciplinary teams, including doctors, nurses, and allied health professionals, to identify patients suitable for the Virtual Ward, identifying clinical needs and liaising with the VW Hub coordinators.
3. Be able to consider a range of factors regarding service capacity/occupancy and consider this when reviewing potential patients to meet clinical priorities/consider acute flow pressures/VW availability.
4. Work together with the VW Hub to complete onboarding processes and handover care and escalation plans.
5. Coordinate necessary equipment, medications, and any care/AHP support to facilitate timely and efficient discharges onto the Virtual Ward caseload.
6. Provide education and support to patients and their families on the VW service.
7. Ensure accurate documentation of patient records, discharge plans, and communications with healthcare professionals.
