These posts are ideal for nurses, physiotherapists, occupational therapists, and other AHPs to join a new and innovative service that focuses on development, innovation, and compassionate care.
We are looking to recruit motivated, caring, compassionate, and experienced Registered Nurses in Adult Nursing or HCPC Professionals to join this new and exciting service within Bassetlaw, based at Retford. You will be working as an accountable and autonomous practitioner alongside the integrated care teams and acute Trusts to support discharge and prevent hospital admission.

You will work in a large and supportive multi-disciplinary clinical team with a dedicated focus on excellent patient care and innovation, providing clinical triage and navigation, urgent response assessments, and facilitation of discharge from the acute provider, focusing on the hospital avoidance agenda.
As a Clinical Assessor, you will:
1. Analyse and develop treatment plans working collaboratively within a multidisciplinary framework and modify as necessary, providing compassionate, realistic, and timely support to a diverse patient group and their families.
2. Undertake holistic assessments of health and social care needs and top-to-toe physical health checks, incorporating the use of a stethoscope, percussion, and palpation techniques.
3. Prioritise factors involved in the clients' presenting condition through clinical reasoning based on knowledge of current professional opinion, client's prognosis, and other physical, social, emotional, and mental health issues.
4. Manage a caseload of patients with moderate to complex needs with supervision and guidance from more senior clinical staff, supporting our patients with their rehabilitation and reablement.
5. Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful, and approachable.
You must be a Registered Nurse (RGN or RN (Adult)) or Allied Health Professional, with demonstrable relevant clinical experience and evidence of continuing professional development.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Qualifications
1. Registered Nurse (RGN, RN (Adult)) or Allied Health Professional
2. Degree level qualification, or equivalent experience and knowledge in a health-related subject.
3. Qualification in care of older people, or relevant experience.
4. Evidence of recent continuous professional development.
