You’ll play a key part in delivering positive outcomes for individuals while contributing to the development and improvement of our services. Key Responsibilities: Provide direct support to service users in line with their Personal Outcome Plans. Undertake case coordination duties, advocating for the needs and interests of service users. Work effectively as part of a collaborative team. Communicate clearly and effectively with service users and colleagues. Contribute to team learning and service improvements. Continue developing your skills to meet SSSC registration requirements. Carry out housing management tasks as required by the service. Participate in varied shift patterns, including evenings, weekends, waking nights, sleepovers, and lone working.
