NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs. Devon Partnership NHS Trust is responsible for the decision making and commissioning processes, working closely with the Local Authority's, on behalf of Devon ICB. Successfulcandidates will have excellent communication and assessment skills; be resilient,able to work autonomously but also work as part of a team. This is a communitybased post and although the role is based in Exeter and East Devon, post holderswill be expected to be able to travel covering the whole of Devon, andoccasionally out of the county. The team values arole which is fast paced and employs the use of holistic nursing where we lookat the needs of the person. This is a role for someone who is invested in supporting others to improve their health outcomes and quality of life using Best Practice. We are an innovative team who likes to use ideas to continually improve practice and work in partnership with other agencies to achieve this., Please see theattached Job Description for further information. The post holder will be expected to undertake assessments, design care packages and procure services for individuals with a learning disability who have complex health needs. These include the CHC process and Section 117 aftercare. The post holder will be expected to carry a caseload of people with complex physical and mental health needs, care manage, review and monitor their packages of care at least annually unless earlier re-assessment is required. The post holder will be expected to have good knowledge of the Mental Capacity Act (MCA) and be able to undertake both MCA and Best Interest decision making. The post holder will be part of a multi-disciplinary team, contributing to the effective provision of services to people with a learning disability with complex needs across the MDT ensuring that other teams in wider LD services are supported when needed. All staff working within the Learning Disability service are responsible for providing and maintaining high quality supportive relationships which at all times acknowledge the individual rights and personal dignity of the service user, encouraging individual opportunity for growth, physically, mentally and emotionally. In the performance of all duties and responsibilities the employee will be expected to show an in depth range of clinical experience and expertise in the field of Learning Disabilities coupled with an awareness of the entitlements of the service user.
