The successful postholder will provide a specialist resource as a Clinical Pharmacist Practitioner for our patients working with clinicians, nursing team and in collaboration with other members of the practice multidisciplinary teams. You will work within our practice community, as an autonomous practitioner providing holistic care whilst assessing, diagnosing and providing treatment to patients. You will also assess, diagnose, treat, refer or direct to other services; promote self-care and educate service users to empower them to make informed choices about treatment.

Main duties of the job
The Advanced Specialist Interest Pharmacist will act as the expert pharmacist for the given specialist area, drawing upon extensive post qualification experience working with patients and the multidisciplinary healthcare team in primary and secondary care where relevant and with community care providers. They will be suitably qualified as an independent prescriber and will have in-depth knowledge of medicines and applied therapeutics.

* Master's degree in pharmacy or equivalent
* Registered Pharmacist with GPhC
* Independent prescriber

Salary: £46,148 to £52,809 a year based on full-time hours.
