We are looking for a dedicated Senior Enablement Assistant to join us on our Kite Unit at the South of England Rehabilitation Centre (SERC).
The Kite unit is a ten-bed neuropsychiatric rehab service for adults following an acquired brain injury, whose impairments are largely in the cognitive, behavioural, or mental health spectrum, with the aim to enable patients to rebuild their lives after a brain injury. We are proud to have been accepted on the pathway towards accreditation with the Quality Network for Neuropsychiatry Services.
You will be responsible for meeting the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. We are passionate about patient centred, culturally sensitive and evidence-based care which promotes independence.

Main duties of the job
* Assess patients, plan programmes of care.
* Provide direct Enablement assistance for the patient to achieve best health outcomes.
* Ensure use of appropriate knowledge and skills to assess, plan, implement and evaluate patients' health needs as well as actively promote health and wellbeing.
* Contribute to effective discharge planning and/or referral processes to health care providers.
* Manage care of patients on a day-to-day basis, working under the leadership of the Nurse in Charge.
* Accurately document assessments of patients on the Trust Clinical Records System.
* Educate and support new starters, students and support for the wider team.
* Role model culturally safe practice and report poor practice when observed.
* Practice in accordance with legal, ethical, culturally safe, and professional standards including the Code of practice.
* Act as a key worker.
* Plan and coordinate care team and planning meetings.

Person Specification
Qualifications
* GCSE in Maths and English or equivalent.
* Qualification at Level 3 in Health and Social Care
* Care Certificate
* Qualification at Level 3 in Health and Social Care or other relevant subject.
Experience
* Fitness to undertake and pass Prevention and Management of Violence and Aggression (PMVA) training as required for role.
* Experience in working in a healthcare setting.
* Experience of working in an environment where clients demonstrate challenging behaviour.
* Full and valid driving license.
Additional Criteria
* Excellent written and verbal communication skills.
* Knowledge and understanding of confidentiality and information governance.
