First line manager with supervisory responsibilities The Assessment & Intake Team is a vibrant and fast paced team based in the community and provides an initial assessment function for the Older Persons and Physical Disability Service. The team works carries out short term pieces of work – up to 6 weeks. The team complete Care Act Assessments put in place long term packages of care, undertake carers assessment, urgent assessments for care and respite, assess for placement in residential and nursing home settings, jointly undertake some DSTs with nurse assessors, complete mental capacity assessments and undertake the initial concern phase of adults safeguarding before passing any enquiries across to the case management team.
What we are looking for in a Senior Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
