Due to retirement, we have an excitingopportunity for a Practice Nurse to join our multi-disciplinary team working across the week between the hours of 0830 and 1830, and ideally one Saturdaymorning in 3 as part of a rota (we are open one Saturday a month). A qualification in diabetes is desirable.

Main duties of the job
The post holder will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives of Bramblehaies Surgery. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and be a valued member of the nursing team.
The post holder would be able to run a full Practice Nurse Clinic. This would include but not limited to providing general nurse treatment room cover and latterly Chronic Disease Management.
The post holder will demonstrate safe, clinical decision making, assessment and expert care.
As a Practice Nurse, the applicant will be required to maintain accurate patient records, ensure accurate completion of all necessary documentation associated with patient health care, and contribute to the preparation of any practice development plans.
The successful applicant will always promote a professional approach by attire and attitude to patients and staff, both in and out of the practice. They will also be required to maintain their own continuous professional development and ensure their PDP is kept up-to-date, and in-line with the requirements of the practice.

The following are the core responsibilitiesof the Practice Nurse. There may be on occasion, a requirement to carry outother tasks; this will be dependent upon factors such as workload and staffinglevels:
a.To develop, implementand embed health promotion and well-being programmes
b.To implement andevaluate individual treatment plans for chronic disease patients
c. To identify, manage and support patients at risk of developing long termconditions, preventing adverse effects to patients health
d. To provide routine nursing care to patients as required in accordancewith clinical based evidence, NICE and the NSF
e.To provide wound care(Doppler etc.) to patients
f.To provide travelmedicine advisory services
g.To request pathologyservices as necessary
h.To process pathologyresults as required
i. To provide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
j.To maintain accurateclinical records in conjunction with extant legislation
