The Optalis Short-Term Support and Rehabilitation Team (STS&R) provides reablement care and support to people who have recently been discharged from hospital. Reablement is a short-term, goal-oriented support service aimed at helping people regain their independence after an illness, injury, or hospitalisation. Instead of doing tasks for someone, reablement professionals work with individuals to build or rebuild skills so they can manage daily activities on their own, such as dressing, cooking, or moving around safely. The focus is on empowering people to maintain as much independence as possible, improving their confidence and quality of life.
STS&R is a multi-disciplinary team consisting of Occupational Therapists, Physiotherapists, and Rehabilitation Care Assistants providing support to customers across the Royal Borough of Maidenhead and Windsor.
Main duties of the job
We are looking for a qualified Occupational Therapist who is registered with HCPC and has experience of rehabilitation and supporting people in the community.
The aim is to support the local NHS communities team with discharging patients back to their own homes and into local care settings. This role will include assessing patients and living conditions, providing equipment, guidance and making suitable adjustments to allow them to continue living as independently as possible. You'll work alongside our team of OTs and Physiotherapists, as well as support staff from the NHS, to help agree a person-centred support plan that can be delivered by our team of Rehabilitation Care Assistants.
The role is based out of Maidenhead Town Hall and will include working in the community, visiting customers at various locations across the borough. It is essential that you are able to drive and have your own vehicle.
