We have an exciting opportunity for a Band 3 Mental Health Support Worker to join this dynamic, expanding team. Bury Home Treatment is extending their hours to be a 24hr, 7 day a week service.
The post holders will work predominantly nights as part of the team to deliver an effective and efficient response to people who access the service.
Bury Home Treatment Team (BHTT) is a 7 day a week service working with males and females of adult working age (18-65) with mental health conditions in an acute stage of their illness. BHTT works as an alternative to a hospital admission in an attempt to provide intensive support to patients within the community and prevent hospital admissions, this includes clozapine titration.
We are also able to screen inpatients with a view of supporting and facilitating early discharge from an acute inpatient setting.
We work alongside patients and their regular care providers (CMHT, EIT etc) for anything from first assessment up to four weeks if clinically indicated.
BHTT is a collection of registered mental health nurses, support workers, a social worker, occupational therapist, clinical psychologist, pharmacist, and consultant psychiatrist along with a junior doctor.
The post holders will work as part of the Bury Home Treatment Team, to undertake allocated duties within the team. You will work alongside mental health practitioners, supporting patients, families, and carers in their own home and also supporting patients who are on the supported discharge pathway from the wards ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.
We will be working 7 days a week, 24hrs a day including weekends and bank holidays.
