Who You Are You are a dedicated and adaptive professional with a background in Health and Social Care, skilled in assessment and communication, both verbally and in writing. You demonstrate an understanding of the Care Act 2014 and are eager to expand your knowledge in relevant legislation, like the Mental Capacity Act 2005. You work well under pressure, valuing diversity and anti-discriminatory practices, while effectively using information technology. You are comfortable working independently, flexibly, and with others, including undertaking home visits. A relevant qualification in social and health care is preferable but not essential, though progression to a higher grade requires successful completion of qualifications such as NVQ3. You are mobile, able to visit various locations around the borough, and can manage challenging family dynamics. Your experience in working with Adults and Carers across different agencies allows you to provide personalized services, ensuring greater choice and control over care and support needs. What the Job Involves As a Care Manager Assistant, under direction and guidance, you will conduct assessments of Adults and Carers in line with Care Act 2014, preparing individual support plans and promoting direct payments to offer choice and control. You'll work creatively to deliver cost-effective services, monitoring, and reviewing support plans for their effectiveness. Collaborating with team colleagues and partners, you ensure service users' needs are met, assisting qualified staff with practical tasks, maintaining records, and engaging consistently with anti-discriminatory practice. You will be involved in safeguarding enquiries and respond to the operational needs as required. Your role extends to service development, providing information and ideas for future services, and participating in complex care pathways. Additionally, you ensure compliance with equal opportunities policies, offering the specified standard of service to clients, identifying shortfalls, and implementing improvements. This role is short term, approximately 100 days, within the Penge & Beckenham Locality Team, focusing on Adults, Older People, and Carers.
