About the Job:
ALL CARE and COMCARE home health provider group has an immediate opening for a FULL-TIME (field-based) California-licensed Medical Social Worker to work as a home visiting social worker to cover Los Angeles, CA and its neighboring areas. Preferably looking for a social worker who is competent and experienced in assessment/charting, a fast learner, and has good communication skills.
This opportunity is also open to an experienced MSW and a self-starter in home health. We welcome highly motivated and positive-minded MSW who are willing to pursue the appropriate training to hone your home health practice. Responsibilities:
1. Assess and provide psychosocial support and resource coordination to patients receiving home health services, focusing on the social and emotional aspects of their illness, assisting with challenges related to their care, and connecting them with necessary community resources to optimize their well-being at home.
2. Conduct comprehensive psychosocial assessments to identify patients' needs, including their social support system, financial situation, emotional state, and potential barriers to care.
3. Maintain clinical records on all patients referred to social work.
4. Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
5. Maintain collaborative relationships with organization personnel to support patient care.
6. Coordinate and direct the home care patient's care based on individual patient needs.
7. Care for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.
8. Educate patients, families, caregivers, and community providers to safely perform care.
9. Assess the home environment for safety, infection control, and community resource needs.
10. Collaborate with the interdisciplinary healthcare team to develop individualized care plans that address patients' social and emotional needs, incorporating them into their overall treatment plan.
11. Provide individual and family counseling to patients and caregivers regarding coping mechanisms for illness, grief, stress management, and decision-making related to healthcare.
12. Identify and link patients to community resources such as financial assistance programs, transportation services, support groups, and other social services to address needs beyond medical care.
13. Educate caregivers about the patient's condition, providing strategies for managing care at home, and offering emotional support to caregivers.
14. Maintain detailed documentation of patient assessments, interventions, care plans, and communication with other healthcare providers.
15. Case coordination.
16. Attend team in-services.
Qualifications:
1. Master's degree in Social Work (MSW).
2. Licensed or certified as required by the state.
3. At least 1 year of social work experience in a healthcare setting.
4. Experience working with individuals facing chronic illness, disabilities, or end-of-life care.
5. Strong assessment and problem-solving skills.
6. Excellent communication and interpersonal skills to build rapport with patients, families, and healthcare team members.
7. Ability to navigate complex social systems and identify appropriate community resources.
8. Knowledge of healthcare practices and regulations.
