Job responsibilities
Workingunder the supervision of the Lead Nurse, the Practice Nurse will work as part of the Nursing & Healthcareteam providing General Medical Services to the patients of Woodlands FamilyMedical Centre. Responsibilities are to:
Assess, plan, develop, implementand evaluate programmes to promote health and well-being, and prevent adverseeffects on health and well-being
Implement and evaluateindividual treatment plans for patients with a known long-term condition.
Identify, and manage asappropriate, treatment plans for patients at risk of developing a long-termcondition
Advising patients with respectto their continuing nursing and medical needs
Triage (face to face andtelephone) assessment and treat patients wishing to see health careprofessional making and necessary referrals to other members of the primaryhealth care team.
Ensure clinical practice is safeand effective and remains within boundaries of competence and to acknowledgelimitations to the team so that referrals can be appropriate.
Ensure all contacts withpatients are recorded as accurately as possible
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care.
Deliver opportunistic healthpromotion using opportunities such as new-patient medicals.
Provide information and adviceon prescribed or over-the-counter medication on medication regimens, sideeffects and interactions.
Support patients to adopt healthpromotion strategies that promote patients to live healthily and encourageprinciples of self-care.
Assess and care for patientspresenting with uncomplicated wounds.
Perform routine procedures likeear syringing, treating wounds and applying or removing dressings.
Taking urine and blood samples,other swabs and specimens.
Support and advise womenrequesting information relating to family planning needs.
Support and manage health needsof women presenting for cervical cytology consultations.
Recognise, assess and referpatients presenting with mental health needs in accordance with the NationalFramework (NSF) for Mental Health.
Implement and participate invaccination and immunisation programmes for both adults and children.
Undertake community visits forhouse bound patients as identified individually and within own scope ofpractice.
Actively identifying, reporting,and correction of health and safety hazards and infection hazards immediatelywhen recognised.
Advise, support and, whereappropriate, administer vaccinations for patients travelling abroad.
Keeping own work areas andgeneral / patient areas generally clean, sterile, identifying issues andhazards / risks in relation to other work areas within the business, andassuming responsibility in the maintenance of general standards of cleanlinessacross the business in consultation (where appropriate) with other sectormanagers.
Undertaking periodic infectioncontrol training.
Routine management of own team /team areas, and maintenance of workspace standards
Waste management, includingcollection, handling, segregation, container management, storage andcollection.
Spillage control procedures,management and training.
Decontamination controlprocedures, management and training, and equipment maintenance.
Meetings
It will be necessary to attendand contribute to various Practice meetings.
Training and personaldevelopment
Training requirements will bemonitored by appraisal initially after 6 months and then yearly and will be inaccordance with Practice requirements. Personal development will be encouragedand supported by the practice, where appropriate. It is the individualsresponsibility to remain up-to-date with recent developments.
Maintain continued education byattendance at courses and study days as deemed useful or necessary forprofessional development ensuring requirements are met.
Develop and maintain a PersonalLearning Plan
If it is necessary to expand therole to include additional responsibilities, appropriate training will begiven.
Liaison
As well as the nursing teamthere is a need to work closely with reception, office and clinical staff toensure smooth running of the practice, reporting any problems encountered tothe relevant person and ensuring everyone is aware of the different roleswithin the nursing team.
There is also the need toestablish and maintain good liaison with other surgeries and agencies includingsecondary care.
