YOUR ROLE & IMPACT
The mission of Charles River Community Health (CRCH) is to partner with individuals and families to help them thrive and lead healthier lives by delivering comprehensive, integrated, and equitable primary care. The Lead Nurse will provide high-quality patient care while supporting clinic operations and staff development. This role combines clinical responsibilities (80%) with leadership duties (20%), ensuring seamless workflows, compliance, and team growth. Key responsibilities include delivering patient-centered care, triaging calls, and managing clinic flow while coordinating care across community resources and healthcare settings. The Lead Nurse will also support the supervision and development of the nursing staff, providing training and constructive feedback. Additionally, this role involves managing schedules, vaccine inventory, and medical supplies while ensuring compliance with clinical policies, quality standards, and safety regulations. YOUR RESPONSIBILITIES
Lead Responsibilities (20%)
* Supervise nursing staff as delegated by the Nurse Manager.
* Evaluate staff performance and provide constructive feedback to foster professional growth and development.
* Support Nurse Managers in training initiatives and staff development to ensure nursing excellence.
* Assist Nurse Managers with daily operations, including guiding nursing staff, and ensuring adherence to clinical policies and procedures.
* Coordinate and manage nursing schedules to ensure adequate coverage, including making adjustments based on patient needs and staff availability.
* Serve as a backup for scheduled nurses in the event of their absence to ensure adequate coverage.
* Maintain accurate records of vaccine stock levels, oversee proper storage and handling, track expiration dates, and ensure compliance with vaccine management guidelines.
* Monitor and maintain medical supply inventory, place orders for necessary items, and ensure proper stocking and storage.
* Prepare reports, update and maintain patient records and databases, and assist with patient intake when needed.
* Ensure compliance with health regulations, safety standards, and clinic protocols.
* Participate in quality control activities and identify opportunities
Staff Nurse (80%)
* Function effectively and safely within the scope of practice of the Licensed Practical Nurse/Registered Nurse to strengthen the practice and provide patient-centered care, working to top of license.
* Provide culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.
* Triage all calls and assist with walk-ins. When organizing care, set priorities based upon acuity.
* Monitor clinic flow and adjust provider schedules to address urgent care needs.
* Demonstrate a thorough understanding of community resources and assist staff, patients, and families in accessing them effectively. Facilitate seamless coordination of care across the continuum, including home care, outpatient clinics, emergency/urgent care, and hospital settings.
* Facilitate the advanced care planning process, assisting patients and families to consider options and goals for future healthcare decisions. Document advanced care goals in the EHR.
* Act as a resource for team members regarding patient care needs, offering guidance and support to both clinical and administrative staff in an approachable and collaborative manner.
* Delegate safely and effectively to support staff tasks that are within scope of practice.
* Communicate clearly with members of the care team regarding patient status and needs.
* Assist in establishing and documenting standard triage protocols, procedures, and patient education information, and present materials for review/approval by medical leadership.
* In conjunction with the nursing leadership, establish and document procedures for treatment room stocking and securing clinical supplies. Ensure supplies are properly stored to enhance efficiency.
* Work collaboratively with other Health Center staff in meeting JCAHO and other regulatory and/or funding requirements.
* Perform other duties as required by Nurse Manager or designee.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
* Bachelor of Nursing preferred.
* Must have Massachusetts LPN or RN license.
* Certification in CPR required.
* Minimum 3 years of experience as a licensed nurse required.
* Excellent communication and documentation skills required.
* Must have knowledge base including basic health teaching, family planning education, and how to care for common pediatric and adult illnesses.
* Must be willing to educate and mentor clinical support staff as needed.
* Must be willing to work evenings as needed and one Saturday every two months.
* Must have basic computer skills preferred or willingness to develop these skills.
* Willingness to support training needs for students and internships as requested.
* Experience with telephone assessment and crisis management preferred.
* Must have a willingness to work flexible hours to meet the organization's needs/demands.
* Must be able to travel to either Charles River Community Health site (Brighton and Waltham) as needed.
* Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
* Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
