Job summary We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team to treat patients using their expert knowledge of medicines. The pharmacist will support and work alongside the practice team including a pharmacy technician for additional support. The pharmacist will undertake clinical structured medication reviews to proactively manage patients including those with complex polypharmacy, potentially developing a specialist area of chronic disease or pain medicines management (such as hypertension/diabetes) The pharmacist will provide primary support to the practice staff for prescription and medication queries. They will support practice repeat prescription systems and lead medicines reconciliation on transfer of care ensuring safe systems for medicines. They will provide expertise in clinical medicines advice while addressing both public and social care medicines needs of patients in the GP practices. This post is to cover the maternity leave of our existing clinical pharmacist and can be hybrid working. Main duties of the job Conduct comprehensive medication reviews for patients, ensuring safety and efficacy of prescribed therapies. Collaborate with physicians and nursing staff to develop and implement patient-specific medication plans. Monitor patient responses to medications, adjusting treatment plans as necessary based on clinical assessments. Educate patients and their families about medication use, potential side effects, and adherence strategies. Maintain accurate records of medication administration and patient interactions in compliance with regulatory standards.
