About the Role
We are seeking a dedicated individual to join our team as a Care Coordinator in Sudbury. In this role, you will work collaboratively to ensure the personal care of our service users is upheld with dignity and respect while promoting their independence.
Your responsibilities will include delivering high-quality care services, maintaining the physical environment of service users, and supporting their daily activities within their homes. You will also oversee care shifts and ensure the wellbeing of both staff and service users in the absence of the Home Care Manager.
Reports To: Branch Manager
Key Duties and Responsibilities
1. Uphold the dignity and standards of service users and their homes.
2. Work efficiently as part of a team to ensure smooth service operation.
3. Participate in training, meetings, and supervisions for professional development.
4. Comply with Health & Safety policies and procedures.
5. Represent the care service professionally and positively.
6. Maintain confidentiality regarding service user information.
7. Support service users' personal development in independence, skills, rights, and choices.
8. Ensure rotas are completed and available to staff on time.
9. Train and assess staff to manage and administer medication safely.
10. Meet with service users to ensure their care needs are met as per their individual care plans.
Skills and Attributes
1. Minimum of 2 years' experience in a care setting with vulnerable individuals.
2. Willingness to undertake necessary NVQ or additional training.
3. Empathy, understanding, and experience in meeting the needs of service users.
4. Strong time management skills and ability to work well under pressure.
5. Ability to make balanced, clear, and timely decisions.
6. Proactive approach to involving others in decision-making processes.
7. Commitment to health and safety practices.
