Please refer to the attached job description and person specification for full details of responsibilities. The post holder will hold a case load of patients suffering with severe and enduring mental illness, working towards the recovery model to support patients to achieve positive outcomes.
The post holder will provide a good role model, develop and maintain good systems of communication, and be able to communicate with a wide range of people in various situations. This includes reporting verbally and in writing regarding clients' progress, assessments, and care planning to the wider multi-disciplinary team.
Maintain accurate and complete records, discussing care planning, assessments, and treatment with clients and their families/carers. Acknowledge the barriers to effective communication and adapt communication style accordingly. Maintain professional standards in line with NMC code of conduct and Trust Nursing Strategy.
The post holder will monitor and maintain the health and security of self and others, managing identified risks and ensuring that health and safety legislation is adhered to. Manage emerging situations, taking appropriate action.
Report actual and potential incidents and attend Trust mandatory training. The post holder will contribute to the continuous improvement of services by identifying the implications of direction, policy, and strategy on current practice in a constructive and timely manner.
Actively involve service users and the public in service improvements. Timely investigations and responses will be made when dealing with PALs issues in line with Trust policy.
