Job summary We have an exciting opportunity We are recruiting a Care Navigator to join our busy, friendly reception team. The successful applicant will be highly motivated, adaptable and be able to work under pressure. Our Care Navigators work closely with our clinical team and wider colleagues in person and via email and tasks, therefore excellent communication skills and strong I.T skills are essential for the role. Experience working in a GP Surgery is essential. The position is permanent and subject to a 6-month probation period. Hours would be set but flexibility is essential as there would be a requirement to provide annual leave/sickness cover when needed. The ideal successful candidate will be flexible to cover holidays, absences and undertake any administrative works to suit the needs of the practice while delivering an empathic and professional service delivery to patients. We are an established team with vast experience of dealing with patients and clinical queries. We have two very experienced GP's, Practice Nurses, on site clinical Pharmacists and many other Healthcare professionals. We are also part of the Primary Care Network (PCN) encompassing 8 local General Practices.
