About the Position
The Home & Community Senior Support Worker role combines hands-on client support, mentoring, and administrative duties to ensure high-quality, person-centered care.
Reporting to the Case Manager, this role serves as the key contact for Support Workers, providing guidance, filtering queries, and escalating critical matters as needed. You will also assist in maintaining NDIS compliance and contribute to client documentation.
In addition to administrative responsibilities, you will provide direct client support, including personal care, domestic assistance, medication management, transport, meal prep, shopping, skill development, and community participation.
A key focus is mentoring and supporting Support Workers, fostering a skilled, engaged team, and enhancing the overall quality of care provided.
Responsibilities
1. Client Documentation & Compliance – Assist with risk assessments, maintain accurate client records, and ensure compliance with NDIS and Aged Care regulations.
2. Client Support & Advocacy – Coordinate essential services, promote independence, and facilitate community inclusion for clients.
3. Monitoring & Evaluation – Regularly assess client progress, adjust care plans with Case Managers, and ensure necessary support interventions.
4. Incident & Risk Management – Handle complaints, manage incidents, implement risk mitigation strategies, and ensure service safety.
5. Stakeholder Engagement & Process Improvement – Collaborate with internal teams (IT, finance, CRM) to streamline workflows and enhance service delivery.
6. After-Hours Support – Participate in rotating after-hours support and ensure smooth client handovers with the Home & Community team.
7. Service Delivery & Workforce Support – Provide direct client support, ensure cultural inclusivity, and assist in workforce coordination for high-quality care.
About you:
* Experience: 2+ years in a Senior Support Worker or Team Leader role within the NDIS sector.
* Qualifications: Certificate or Diploma in Community Services, Disability, Mental Health, or related field.
* Industry Knowledge: Experience in home and community care, with a strong understanding of NDIS processes and compliance.
* Certifications & Requirements: Driver’s license, First Aid, NDIS Check, and WWCC (Working With Children Check).
* Communication & Interpersonal Skills: Exceptional customer service, rapport building, and negotiation skills.
* Leadership & Mentoring: Proven ability to lead, mentor, and support teams in a person-centered environment.
* Resilience & Problem-Solving: Strong interpersonal skills with the ability to work under pressure, meet deadlines, and manage competing priorities.
* Policy & Compliance Awareness: Ability to interpret processes, policies, and restrictive practice guidelines while maintaining professional boundaries.
