You’ll conduct home visits to lead holistic assessments, plan and review interventions. You’ll provide support across our multi-disciplinary Reablement Service, a strengths-based short-term service helping the people we care for maintain or regain independence in daily activities.*What you’ll be doing:** Leading on the assessment, planning and reviewing of complex case work.* Conducting person-centred functional analysis and interventions to help patients maintain or regain independence in daily activities.* Collaborating with a diverse team, including Occupational Therapists, practitioners, carers, sensory staff and business support.* Working independently, providing expertise and support to staff, informed by social care legislation and clinical knowledge.* Our service operates seven days a week and there will be a requirement to work weekends on a rotational 1:8 basis.*What we’re looking for:** Occupational Therapy qualification and professional registration with HCPC.* Experience in assessment, planning and reviewing of case work.* Ability to work independently and collaboratively, escalating cases as required.* Commitment to demonstrating our values and behaviours. * An understanding of the statutory duties of local government in social care and the social care model.*Why join us?** Join the largest care provider in Hampshire.* Opportunities for experienced and newly qualified Occupational Therapists.
