Health Care Assistant Rapid Response - Community Role based at Talbot Park - Timaru
Shifts: Monday to Sunday, 0800-1630 or 1230-2100
Contract: Fixed Term Part Time to October 30th, 2025, 0.8FTE (64 hours per fortnight)

About the Role
We are looking for a Healthcare Assistant with community experience to join the Community Hub Rapid Response team. This service provides a multidisciplinary approach to patient care focused on hospital avoidance, early supportive discharge, and non-acute rehabilitation in a home care setting.
About the Team/Service/Location
The HCAs in this team work under the delegation of registered health professionals and provide care and support to patients and clients in their homes.
About You
This role will suit someone who can work with a wide group of healthcare professionals, practice independently, and use prior expertise to ensure patient and client-centered care, enabling them to maximize independence. Other requirements for success in this role include:
1. Previous community experience and/or a passion for providing care that supports people in their own homes
2. Ability to work independently and use initiative to assess and report changes in health status of clients
3. Delivery of a safe and effective patient-focused service
4. Ability to work with patients and their whānau and family
5. Must have a current full New Zealand Drivers Licence
6. Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
7. Experience in projects/initiatives that effect Te Tiriti principles and frameworks, particularly in healthcare settings

Salary Range
This role is covered by the terms and conditions of the Nursing Collective, and the salary will be in accordance with the Pay Scale, depending on relevant experience.
