Community Practitioner - Discharge to Assess/Intermediate Care at Home
The Discharge to Assess team is looking for a Registered Nurse to fulfill an exciting community maternity leave cover opportunity within Stockport Foundation Trust Community.
We need people who share our excitement, embrace different ways of working, and are able to work flexibly across organisational boundaries. An opportunity has arisen for a Registered Nurse - Band 6, to work flexibly within the Discharge To Assess team.
The Discharge To Assess element is a multi-professional service that assesses patients upon discharge from the hospital in the community. We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service: Health, Adult Social care, or partner agencies.
Main Duties of the Job
The Discharge to Assess element supports people who are clinically optimised and do not require an acute hospital bed but may still require care services with short-term, funded support to be discharged to their own home or another community setting.
Assessment for longer-term care and support needs is then undertaken in the most appropriate setting and at the right time for the person. We provide holistic multidisciplinary interventions and support to stabilise patients in their own home or usual residence. This comprises a multi-disciplinary integrated team.
The Rehabilitation Element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admissions, enable early discharge from the hospital, and prevent unnecessary admission to long-term residential care. This role will require the post holder to be flexible to cover all areas within the Discharge to Assess service, covering 7 days from 08:00-21:00.
Job Responsibilities
The post holder will be a qualified Registered Nurse. You will be working within a multi-professional team incorporating Physiotherapists, Occupational Therapists, Nurses, Social Workers, and Rehabilitation Support Workers, as well as liaising with other community health and social care teams.
The whole service provides a person-centred approach, which can be provided in a client's own home. The service is provided across 7 days a week covering core hours of 08:00-21:00.
You will be based at Regent House, Heaton Lane, Stockport, SK4 1BS. The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team. You will be offered 1:1s and access to supervision peer support. The service promotes regular profession-specific and MDT team meetings.
You will need to be a Registered Nurse with an enthusiasm for working within the community, with the ability to work flexibly between all aspects of the service. We place a great emphasis on training and personal development and will provide you with access to training and supervision. The candidate will be able to demonstrate sound clinical reasoning, excellent time management, and advanced communication skills. There will be support available from clinical leads. Lone working is an expected element within this community role with support from the Multidisciplinary team.

If successful, this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary.
In addition to this, it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
Person Specification
Education and Qualifications
* Degree in Adult Nursing
* State Registration with the NMC
* UK driving licence (will need use of a car for work purposes and business use insurance on commencement of post). Driver with daily access to a car
* Evidence of Continuous Professional Development maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations
* Related academic attainment
Knowledge
* Knowledge of community practices and a wide range of conditions- theoretical, practical, and professional
* Knowledge of the principles of clinical governance
* Health, safety, and risk awareness. Knowledge of Risk Assessment and management.
* Understanding of professional ethics and their application and practice.
* Knowledge of health legislation and current practice. Knowledge of Mental Capacity Act and application
* Knowledge of Safeguarding responsibilities
* Knowledge of working in community services
* Completion of recognised relevant postgraduate courses.
Experience
* Experience of working as part of a multidisciplinary team.
