Main duties of the job
The Role
As a Frailty Practitioner, you will play a critical role in our new multidisciplinary team, covering one or more of the 6 new frailty virtual wards we have planned for our localities. The post holder will work closely with consultants, advanced clinical practitioners, pharmacists, specialist nurses, and other healthcare professionals to ensure seamless, integrated care on the virtual ward, by providing and supporting specialist assessments and interventions for those admitted to the Frailty Virtual Ward.
We will consider applications for people seeking full and part-time posts and flexible hours. We welcome applications from experienced band 6 nurses or registered Allied Health Professionals, with a rounded knowledge of managing older patients with various presentations and long-term conditions.
Key Responsibilities

Job responsibilities
The post holder will be a specialist in frailty for the Community Virtual Frailty Ward. This includes physical assessment and clinical reasoning, diagnostic reasoning, and the ability to identify issues with polypharmacy in frailty to action the necessary medication review. Using these skills will assist in the recognition of early symptoms of frailty, disease exacerbation, acute illness, and injuries.
An autonomous practitioner who will provide expert clinical case management for frail patients admitted to the frailty virtual ward with acute/intensive medical needs and at risk of further deterioration in health that may result in avoidable hospital admission or unnecessary length of hospital stay. Be responsible for facilitating efficient, effective coordinated assessments and treatment for frail older adults.
Undertakes specialist frailty assessment, plans and implements care in a variety of settings, provides highly specialist advice to patients/clients, carers, relatives, and other professionals; maintains associated records.
Develops and delivers specialist frailty education programmes to service users, families, and professionals.
Key member of the Ageing Well Multidisciplinary Team (MDT), and lead practitioner to support Livewell community integrated teams in completion of comprehensive geriatric assessment, working in conjunction with primary care network colleagues.
The role reports to the integrated operational lead for the frailty virtual ward and links closely with the practice, strategic and team leads in AFSS.
The role provides specialist frailty assessment, education, and training for professionals and people using our services. It supports the enhanced primary care agenda through the use of comprehensive geriatric assessment. The role supports service transformation and delivery for frailty.

* Ability to work across community locations in Plymouth.
Knowledge
* Knowledge of Community Services/Third sector services suitable for frail patients.
* Knowledge and understanding of legislation relevant to practice.
* Experience of using electronic patient/service user record systems.

Qualifications
* Registered practitioner.
* Professional knowledge acquired through degree supplemented by post-graduate diploma with 3-5 years significant relevant experience.
* Specialist training and experience, short courses plus further specialist training.
* Membership of the relevant Professional Body.
* Evidence of recent professional development in an up-to-date portfolio.
* Leadership and management qualification/training or evidence of experience in this area.
* Independent prescriber (once fully competent in role).
* Teaching qualification.
* Additional clinical qualifications in Frailty/Older Persons Care.
