PROGRAM PURPOSE:
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control. NACDD promotes health and reduces the burden of chronic disease through various programs and initiatives in collaboration with the Centers for Disease Control and Prevention’s (CDC) Division of Nutrition, Physical Activity, and Obesity and the Division of Population Health.
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
* Support activities for landscape scans related to nutrition security and safe access to physical activity, including conducting research on local partners, supporting an inventory of local programs/services, and analyzing local data to identify priority populations.
* Build and strengthen HPCDP relationships with key organizations working on nutrition security and safe access to physical activity through partner meetings and maintaining a partner inventory.
* Contribute to the development of the nutrition and physical activity portions of HPCDP’s strategic plan, including facilitation of partner engagement meetings and researching policy options.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
* Hybrid service hours: 30% on-site, 70% telecommute.
* Service hours are to be completed Monday-Friday between 8:00am and 5:00pm Pacific time.
* A maximum of 20% of service hours may be training, education, or other approved activities.
* A maximum of 10% of service hours may be fundraising activities.
DESIRED SKILLS & COMPETENCIES:
* Bachelor’s degree or equivalent relevant experience with at least two years in related fields, preferred.
* Proven ability to work cooperatively, manage complex projects, and build positive relationships.
* Ability to work independently and as part of a team.
* Strong time management, project management, and organizational skills.
* Strong written and verbal communication skills.
REQUIRED COMMITMENT:
* Service members are expected to complete 1200 service hours within one year.
* Service year is anticipated to begin 02/03/2025 and end 02/02/2026.
ORIENTATION AND TRAINING:
* Members must participate in pre-service orientation, PHA-specific training, program-specific training, and any other necessary trainings.
EVALUATION AND REPORTING:
* Complete quarterly assessments of the PHA Program, monthly reflection forms, and participate in quarterly performance evaluations.
* Submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
* Must be at least 18 years of age at the commencement of service.
* Have a high school diploma or its equivalent.
* Be a citizen, national, or lawful permanent resident alien of the United States.
* Satisfy the National Service Criminal History Check eligibility criteria.
* Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
Must comply with all Host Site vaccination requirements, including for COVID-19.
