POSITION SUMMARY
Registered Nurses provide direct care to patients and are part of the care given by other disciplines. Registered Nurses must continually strive to provide the highest quality of care possible for the patients. They must also lead the nursing team toward the achievement of the goals of the Cuyuna Regional Medical Center. High quality care and overall efficiency are essential components to this success.
Position Qualifications
Education and Experience:
Previous experience in Home Care preferred.
Current Minnesota RN license. American Heart Association BLS certification within 30 days of hire. Valid Minnesota driver's license required.

RN Responsibilities
* Exercises concern for the patient's welfare above all else.
* Remains up-to-date on new nursing skills, technology and requirements in the changing healthcare environment.
* Utilizes the nursing process (observation, assessment, planning, implementation, and evaluation) in the delivery of care.
* Performs duties in accordance with physician orders, Home Health Partnership, the Medical Center's policies, methods, and techniques, and by cooperating with other departments and personnel.
* Practices and maintains high ethical standards.
* When requested, participates in in-service programs, education programs, and orientation of new personnel.
* Provides leadership and promotes teamwork.
* Counsels the client and family in meeting nursing and related needs.
* Responsible for the direct and indirect care of patients assigned to her/him under the guidance of the Director/Assistant Director.
* Maintains accurate and complete records of nursing assessments and care.
* Administers medication to patients; performs treatments.
* Maintains knowledge of patient's condition.
* When requested, assists in the orientation of new personnel.
Home Care/Hospice Responsibilities
* Independently and effectively coordinates and cares for own caseload by: preparing and keeping up to date a written plan of care; assuring financial coverage by following payers prior authorization and requirements; exploring and utilizing available resources.
* Supervises the work of LPN(s), Home Health Aides, Homemaker in the home according to policies, rules and regulations.
* Working knowledge and use of Lifeline equipment.
* Takes initiative in development, organization, implementation and evaluation of a project or specialty area assigned by the Director.
* Demonstrates the ability to triage Home Care/Hospice business intake tasks effectively.
* Demonstrate Standards of Excellence when other duties are assigned.
Working Conditions/Physical Requirements
The RN in Home Health provides essential support to patients in their homes, promoting independence and improving quality of life. This role requires frequent travel between patient homes, necessitating specific physical capabilities. Ability to walk, stand, and move throughout various home environments for extended periods. Frequent transitions between different patient locations, which may include navigating stairs. Must be able to lift, support, or transfer clients weighing up to 50 pounds using safe body mechanics and assistive devices. Capable of performing tasks such as helping patients move from beds to wheelchairs or assisting with ambulation. Ability to sustain physical activity over long shifts, often exceeding 8 hours, including standing, walking, and performing caregiving tasks. Ability to reach for items and assist patients with personal care activities, which may involve bending, stretching, and reaching overhead. Proficiency in using hands for tasks requiring fine motor skills, such as administering medications and assisting with grooming. Ability to lift and transport equipment or supplies (up to 20 pounds) to and from patient homes. Ability to communicate effectively with clients and their families, understanding their needs and preferences. Flexibility to adjust work hours as needed to accommodate patient schedules and emergencies.
