Main duties of the job
As a Community Nurse - Band 5, you will play a pivotal role within the Community Nursing Team, supporting patients with diverse health needs in their own homes. Your focus will be on providing holistic, high-quality, evidence-based care while promoting patient independence and recovery.
You will:
* Conduct first visits, address immediate care needs and report to the Team Leader for further discussion or assessment.
* Provide care according to the available evidence of best practice and the support of others in the development of evidence-based practice.
* Actively involved in promoting self-care for patients and building Health Promotion into care.
* Develop and participate in planned teaching programmes for patients.

* NMC current pin/or about to qualify
